Volunteer Waiver. Please read carefully.
The City of Montgomery is committed to conducting its programs, services, and events in a safe manner and holds the safety of City Volunteers in high regard. Equally, it is important that any Volunteer, or parent or guardian of child volunteering with the City, know the duties and responsibilities of the task they are undertaking, and the Volunteer has the physical and mental capacity to perform such tasks.

With this in mind, by submitting this document I acknowledge:
- I understand the tasks I will be assigned and/or I will undertake, or which my child may be assigned or undertake
- I certify that I, my child, or organization volunteers are in the appropriate physical and mental condition to participate in such tasks as a Volunteer.
- I understand there are certain risks involved in such tasks and I agree to assume all such risks including any damages resulting from physical injuries, death, loss of services or consortium, loss or damage to property, or any other loss which I, or my child, or organization volunteers may sustain as a result of participating as a Volunteer for the City of Montgomery.
- I understand and agree that I may not use any power tools, specialized tools, or operate City equipment or vehicles without the expressed written permission of a City employed supervisor.
- I understand that any injury to myself, my child, or my organization volunteers sustained as a result of volunteer duties may be covered under the City of Montgomery’s volunteer accident insurance as negotiated by the City year to year. Any coverage so provided will be governed by policy language which may provide benefits to me in excess of my personal health insurance coverage. I also understand that the City of Montgomery does not assume any responsibility for, nor obligation to provide, financial assistance or any other assistance to me or my family or child, or organization volunteers including but not limited to medical, health, or disability benefits, in the event of injury, illness, death, or property damage sustained while volunteering for the City. If any injury should occur, it is my responsibility to complete a Volunteer Event Incident Report form.
- As a Volunteer, I, my child, or organization volunteers agree that we will represent the City and ourselves appropriately in how we perform any tasks as a Volunteer, how we dress for the task, and interact with the public.
- I also agree that I will report any inappropriate behavior by another volunteer, a City employee, or person attending the event to the City event coordinator as soon as possible to allow the City to intervene and take action.
- Finally, I grant full permission to the City to use any photographs, videos, or recordings of myself, my child, or the minor for whom I am the legal guardian while volunteering for any purpose.
Additional Organizational Volunteer Waiver.
In addition to the above, as an Organization Volunteer:
- It is important that any Volunteers working through your organization know the duties and the responsibilities of the task they are undertaking.
- By signing and accepting this Organizational Waiver, you agree that Your Organization shall be responsible for scheduling and supervising Your Organization’s Volunteers participating in this City event. City staff may assist you in identifying your staffing needs and defining the task required of your Volunteers. However, the ultimate responsibility for the supervision and training of your Volunteers rests with Your Organization.
- You agree that Your Organization will only assign tasks to your Volunteers which are age-appropriate and which your Volunteers are physically and mentally capable to handle.
- On behalf of Your Organization, and on behalf of your individual Volunteers, you waive any claim against the City for any injury or illness, including death, which may result from volunteering at this City event. Further, Your Organization agrees to hold harmless and indemnify the City against any claim which may be asserted by one of your Volunteers as a result of any injury or illness sustained, including death, as a result of participating in this City event.
- It is understood and agreed that if individual Waivers are required for your Volunteers, you will secure those Waivers, as necessary, to provide the appropriate protection, waiver, or release to Your Organization.
